5. ENTRUSTMENT OF HANDLING

In the course of our business operations, there may be instances where we entrust some or all of the handling of personal data to a third party, and this will be done within the defined scope of the purpose of use. 2) Confidentiality Obligations for Employees: Employees tasked with handling personal information are bound by obligations to maintain confidentiality. They undergo education and training to enhance their ability to protect personal information. We provide necessary and appropriate supervision to ensure the secure management of personal data.
3) Formulation of Rules for Proper Management: We establish rules for the proper management of documents and information devices containing personal information. These rules are communicated to employees, and we ensure compliance with them.

These measures collectively contribute to the secure handling and protection of personal information.

7. DISCLOSURE OF RETAINED PERSONAL DATA

If you wish to request disclosure, correction, addition, deletion, suspension of use, and suspension of provision to a third party of your personal data held by us, please follow the procedure below:

・Documents that can confirm the agency relationship with the person in question (In the case of a legal representative of an adult ward, a certificate of registered information, in the case of a legal representative of a minor, a certified copy of the resident card, etc.)

2) In the Case of Correction, Suspension of Use, or Suspension of Provision to a Third Party of Personal Data Held by the Individual or His/Her Agent:
Please inform the details of your request to the contact information in Section 11 and provide information necessary for identity verification, such as the requester’s name, address, date of birth, and telephone number, according to the instructions.
3) How to Respond to Requests for Disclosure, etc.:
Regarding requests for disclosure of retained personal data or records of provision to third parties, we will respond by sending a letter to the current address listed in the required documents above, according to the requester’s request. Alternatively, we will respond by attaching the electromagnetic records to an email and sending it to the email address listed on the request application form above. We will respond to requests for correction of retained personal data, suspension of use, and suspension of provision to third parties using our designated method.
4) About Fees:
If you are making a request for disclosure, etc., a fee of 1,000 Japanese yen will be charged at the time of application reception (including consumption tax, etc.).
5) Purpose of Use of Personal Information Obtained in Connection with Disclosure Request Procedures:
Personal information acquired by us through disclosure request procedures will be used for investigation, identity verification, and responses to disclosure requests.
